Job description

MENTAL HEALTH AND SAFEGUARDING EXP REQUIRED - OFFICE -BASED - NON NEGOTIABLE

Experienced level 3 social worker with extensive post qualifying and recent experience in statutory adult social care required. Must have experience in completion of care act assessments, care plan reviews, s117 aftercare, safeguarding within past 12 months. 3 month temporary post being sought to assist adult mental health statutory social work team.

The team supports individuals 18+, transitional cases and handover work with children services also on a case by case basis. The team work with adults in the community, hospital, rehabilitation and care settings within Calderdale and for individuals placed in care settings outside of Calderdale.

The team are separate to the CMHT, working collaboratively with partner agencies where there is a need or request for adult social work intervention. The team is currently experiencing high demand. Temporary agency worker being sought to support the team through a period of high intensity. Agency workers will be expected to be office based (central Halifax), to provide support in responding to urgent safeguarding work, crisis intervention, duty work and hospital discharges. Reporting directly to Team Manager and Team Leader.
