Job description

Our Client Bexley Council, is looking for a Qualified Social Worker to join their Complex Care Team.

The Complex Care Team works with older people, people with physical disabilities, learning disabilities, or mental health problems. We undertake Care Act 2014 assessments and support our residents to identify how they can best live their lives as independently as possible.

  • To work with service users and their carers, to promote and maintain their dignity, choice and independence. To assist as part of the multi disciplinary team in improving quality of life by, devising re-enablement programmes to maximise independence and assist with accessing resources to provide practical help, where appropriate.
  • To give advice and information, and signpost people to appropriate provision in the voluntary and independent sectors.
  • To facilitate the safe discharge of service users from hospital.

What you need:

  • QSW Status - including qualification, Social Work England registration and completed ASYE
  • A minimum of 2 years' experience
  • Right to work in the UK
